One of the most important things to Steve Jobs – and what has led to his company’s success – is their understanding and definition of design. He’s famously quoted as saying that “Design is not just what it looks like and feels like. Design is how it works.” Unlike most companies, Apple doesn’t have engineers build a product and then have designers apply aesthetics; they start with the design and have the engineers build out the system such that the final product not only is aesthetically pleasing, but also exceptional from a user experience point of view. We think much the same way here at SOAPware about the products we build – particularly the electronic health record software product. When most EHR companies develop their system, they start with the question “what data needs to be collected?” Then, database definitions are created, and software engineers begin building the user interface such that it will populate the database. We believe this is entirely the wrong approach, as the final product often ends up convoluted, disjointed, and is a real bear to use – especially for hours upon hours every day. In stark contrast, when the SOAPware EMR was first designed in the 1980s, it wasn’t because we aspired to populate a database with a bunch of clinical information. Charting-OrdersOrder Entry

Simplify the process of submitting orders to internal departments and external laboratories, and automate the process of receiving test results and reports via HL7 interfaces. Over 60 lab companies are already interfaced, and more are being added every month!

Lab, imaging, and immunizations can be entered using the Order Manager, which can keep track of all orders ordered for the patient. Order sets can be created which will allow the user to insert multiple orders at one time. User specific orders designs can be created with clinic information and logos to be printed or faxed directly to the receiving facility. Using the Order Manager to record ordered immunizations allow these to be exported to meet the Meaningful Use Stage criteria.

SOAPware Tempates IconTemplates

In less than 60 seconds you can document an entire note by starting with a template. Most importantly – the template doesn’t limit your input options. Start with a template, and tweak the note so it matches your style, and accurately captures the patient encounter.

Templates within SOAPware are so customizable and flexible that they aren’t even named “templates”. We refer to our templates as “Docuplates” which stands for Documentation templates.

These allow common sets of document items to be saved as a group so that they can be reused later. Similar in nature to templates, Docuplates can contain a pre-formatted SOAPnote for aiding in documenting an encounter. This capability is useful for documenting common conditions or frequently repeated encounters, eliminating the need to recreate the same collection of document items each time. User Tip: Docuplates can be created from almost any document in SOAPware, including SOAPnotes, Flow Sheets, Labs, Drawings, Summary, etc.

SOAPware has a large Cloud Library offering thousands of different docuplates available for practices. Users can even create their own docuplates, whether starting from scratch or using existing docuplates as a jumping off point. Docuplates can be used not only in the visit documentation but also in any chart section within SOAPware. This allows users to create the patient’s chart to best suit their needs. SOAPware Patient Summary allows users to view a “snapshot” of the patient’s overall summary in one place within the patient’s chart. Users may view and document any changes to the patient’s current active diagnosis, prescriptions and allergies. Each of these areas are designed to calculate for Meaningful Use when structured information (SMARText) is inserted into these fields, satisfying the following objectives:

  • Maintain an active medication list
  • Maintain problem list
  • Maintain a medication allergy list
  • Implement drug-drug and drug-allergy interaction checks

Information in these sections may be automatically transferred into the patient’s current encounter or simply displayed as part of today’s visit. Each of our fields within the SOAPware chart allows great flexibility with information and data entry. Additional comments may be attached to a diagnosis with any additional information the user would like. Allergy items contain sub entries where users may document the onset of an allergy, severity, and many other different types of information a physician would require. When an allergy is inserted into the Summary this automatically transfers into the Rx Manager for reference as a clinician is submitting a prescription.

Remember the time when you had control over how you created your documentation? Those days don’t have to be over. Unlike most EHR systems, SOAPware is incredibly flexible. Free-flowing text, custom templates, pick lists, shortcut macros, cloned encounters — it’s all supported.

SOAPware offers several ways of entering data; SMARText, Free text (typing or dictation), Speech Recognition, Scanning in paper encounter forms, and dictation and transcribing.

SMARText: (from manual) Exclusive to SOAPware, SMARText is the backbone for medical documentation using our EMR. SMARText entries replace and extend macros to auto-enter blocks of text data that are specially designed to be searchable by computer.

Because SMARText items contain much more data than plain (free) text, these entries make it possible for items used in documentation to have underlying relationships to other items in the medical record. Using SMARText structured items will prepare clinics for important quality measure reporting linked to Meaningful Use data, as well as to the requirements related to the Patient Centered Medical Home.

Pieces of information or objects, including text, CPT codes, medication items, images, photos, videos, and audio files; can be combined to form SMARText items. Entered using a combination of shortcuts and keystrokes, multiple SMARText items can be placed together to form new and useful structures.

SMARText items are able to be customized to the users preference in wording and appearance.

If SMARText is used to document the patient’s medical history and office visit, SOAPware learns the users preferences in SMARText and will keep them readily available by most used to least used for easy entry.

Some Reasons to Use Structured Data

If the user desires to participate in “Meaningful Use” and “Pay for Performance” (P4P) initiatives, to engage in practice quality improvements, and/or electronic prescribing, then the user will need to use structured data. Note: It is not mandatory that the user use structured documentation in his/her patient charting procedures. However, while using free text may be familiar and seem easier, taking the time to learn more about the benefits associated with using structured data entry could greatly benefit the user’s practice on many levels. Structured data entry not only makes the user more efficient, it will also be more likely to allow the user to receive greater reimbursement in the near future. In fact, it appears those physicians who choose not to participate in Meaningful Use and P4P initiatives will likely be penalized.

SOAPware Import Export IconFULL PDF SUPPORT! Quickly and easily export any or all parts of a patient chart into PDF format, and instantly drag-and-drop PDF, Microsoft Word, images and audio/video files, and more right into the patient chart.

There are two main mechanisms for adding outside documents to SOAPware. In each viewer, there is a Scan button allowing for the addition of documents directly into a patient’s open SOAPware chart. The Document Import workspace is the second option and is the main mechanism for importing documents into SOAPware. Document Importer is the topic for the rest of this discussion. The Document Import workspace is used for importing documents and files from outside of SOAPware directly into patient charts without having to open each, individual patient chart in the Chart workspace. It is especially useful when dealing with large numbers of files spanning multiple charts, such as when the user needs to import documents created during bulk scanning operations.

To access Document Import, Click SOAPware in the menu bar, and Click the Document Import menu item. The workspace in Document Import consists of four vertical columns and the patient name panel at the bottom.

There are 4 main areas contained in the Document Importer

  1. Mini-Chart Rack – The first column/panel on the left is a list of patient charts, arranged alphabetically. This is similar in function to the Chart Rack. The name of the patient selected from this list is displayed in the patient name panel at the bottom.
  2. Folder Browser – The second column/panel is a folder-browsing column/panel and allows for the finding and selection of folders (i.e. folders containing document to be imported). This is the same view that Windows creates when My Computer or Windows Explorer are selected.
  3. Folder Contents – The third column/panel displays the contents of the folder selected in the second column. Right-clicking in this area allows the view or sorting options for the display to be changed.
  4. Chart Sections – The fourth column/panel (right-most) shows the chart sections for the active patient whose name is displayed in the patient name panel. This is similar to Chart Navigator. Click-and-drag the vertical splitter bars between columns to resize or hide the columns.

HL7 data feeds are the most popular means for automatically importing lab results, radiology reports, transcription and other types of healthcare information. Labs sent from outside facilities to SOAPware will display in a patient’s Chart. In addition to the lab values and reference ranges, HL7 labs will contain other information within the report:

  • Patient demographics
  • Collection Date
  • Specimen Source
  • Report Generated Date
  • Performing Facility Information

HL7 Lab Tasks

HL7 lab reports placed in the patient chart will automatically create a task and place item on the ordering clinician’s task list. This can be signed off or reassigned just as any other task. Note attached to task will say “HL7-facility name”.

The HL7 integration allows lab results to come directly from the lab company into the patient’s lab chart section as well as to the ordering clinician’s; a Task List item is then created, alerting them of the results for quick reviewing. Custom flow sheets of HL7 lab results can be created in order to closely track and compare results.
